I got my PR under CEC a few weeks back.
In 2008, I had syphilis which was treated right away, but even if it was treated, it would always show up on the blood work for many many years (that's how it is - which is very normal)
So, it showed up on my blood work during my medicals for my PR, but upon producing the document proof from 2008, my medicals were passed.
Now, with my PR document, I received another document that says "Medical Surveillance Undertaking document".
I am not sure what this document is, so when I was landing I asked the immigration officer, he gave me a Medical Surveillance handout that had a number for Public Health, he told me that I would have to call that number ASAP and tell them that I have landed, he told me that they will guide you on what is to be done.
You have to call that number, which is a Public Health Office.
They will schedule you for testing for certain periods then give you a Compliance Report.
You need to do this ASAP within X days upon landing.

I guess if they're not satisfied that you are "healthy" then who knows what will happen. If you are cured as you say then why worry.
Syphilis is a communicable disease that is considered a public health risk; as such, a condition of your PR is being monitored by the Public Health Office. Just call them and they will explain the process to you.
If you fail to comply with the medical surveillance, it's grounds for being reported, with the potential loss of your PR status. So, don't ignore the process.
