Medical request received for visitors visa

Hi,
My dad has been to Canada multiple times. He recently applied for a new visitors visa (not for super visa) but he got a medical request. In 2014, we had his medical done for a visitors visa extension when in Canada and everything was smooth. I am really confused why he would get a medical request for a visitors visa for a stay duration less than 6 months?
Has anyone come across any such examples before?

Probably because he stayed in Canada for longer than six months during his last visit and required a medical - so this time CIC is asking for one in advance (assuming he may once again end up staying longer than six months).
