Medical insurance for spouse inland sponsorship while waiting?

What is best option for medical insurance for a spouse while waiting for the application processing inside Canada?

Also, if the spouse is from EU and doesn’t need a visa, but needs to extend their stay, do we need to apply for extension, or it will be automatically extended?
