Can someone please explain to me why the online document checklist is stating that a medical is required for TRV?

I was under the impression that for a visit less than 6 months the medical exam was not required.

Also since my wife's upfront spousal sponsorship exam is excessive demand exempt this also means she will need to redo the medical for TRV?

It depends by country.

Some offices accept the same medical for differente process, some don't. Check with your local authority,

Thank you for your reply. My wife is Indonesian and VO is Jakarta. We applied for TRV/Study Permit a few months ago and upfront medical was not on the required doc list. This was an application to take a year long course. Now that we have the GCMS notes and plan to address the concerns and apply for a month long visit, the upfront medical is apparently required for just the TRV when it wasn't for the SP/TRV. This leaves me confused.

Can we upload the completed exam confirmation the panel physician gave my wife for her PR application and wait and see if it is accepted or not?

Yes, you can send the same medical; there will be no issue.

Yes I also uploaded the same medical from my PR application to apply for a visitor TRV, as I answered yes to the question if I had one done in the last 12 months. got the status passed after app transmission and got my TRV already.
