Hello,
Can you please let me know if you received a separate medical form to complete exam for dependent. My husband received form with his name and one blank so he just done medical. Now they ask for dependent exam but i don’t have form with the dependent info. Do we take applicant form and blank to have exam done? Thanks

Yes, all dependents have their own medical form ( with name and emi).
