Thanks for the reply. The issue is that my husband is applying for the EE and I am accompanying person. I am now in Canada and will live Canad in September for 1-2 years. That is why wanted to get tested now. Is it not a good idea?
Your medicals will only be valid for a year, so you may spend more money than you need to getting a second medical later on. You can get medical in other countries too http://www.cic.gc.ca/pp-md/pp-list.aspx

Another issue is that the test results are directly sent by a doctor to CIC, but if there is no account how can it work?

As far as I remember, Dr. does not need an express entry number. He/she will give you an information sheet, which you'll need to upload to your mycic profile, after invitation to apply. At the same time Dr. will enter your medical info into a database, to which CIC has access to.

I think it is a matter of money, because if you don't validate your COPR, or enter Canada as a permanent resident, before August 2017 you'll need to undergo a medical exam again.
