I am taking my medical exam for outland sponsorship from one of the doctors on the panel list later today & I would like to please know if someone can post a link to form IMM 1017E which I need to bring to the doctor. I tried finding the form online but had no luck.

Are you doing an upfront medical exam (before you've sent the application)? Then you don't need that form. CIC mails or emails this form to those who do not do the upfront medical exam.

I am redoing the exam since last years expired & the secretary told me that I have to bring the medical forms. I know I bought the medical forms last year for the first time I did it.

"With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:

IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)"
