Anyone else in the process of or have submitted their application for PR through express entry? I'm wondering when the medical exam is needed, will CIC send you a letter after submitting the PR application? Or will you have to submit the medical exam with your PR application?

You can get your medical exam done after you get an ITA. It is called an upfront medical exam.
I did it upfront once I got my ITA. Some places are fully booked for months depending on where you are.
