MEDICAL EXAM while EXTEND STUDY PERMIT

Hi there,
I had medical exam in May 2013 in Vietnam (before I came Canada). But they didn't give me back any report or form.
Now my study permit is going to be expired on March 2014 and I am trying to extend my study permit on-line and CIC asked me if I had any medical exam in appointed panel physician in last 12 months.

The thing is if I answer YES, they'll require my Medical report. But I don't have any Medical report because panel physician didn't give me back anything.
If I answer NO, they won't require Medical report but in fact, I did have medical exam in last 12 months.

What should I do to solve this problem? Which answer is better? Please help me. Thank you so much!
