Medical exam - which VISA office is processing it? HELP!

Greetings,

On the "U.S Applicant Checklist" in Appendix C, regarding the medical exam, there is a question (#3 to be exact) that asks which VISA office will be processing the file. How exactly are we supposed to determine this? My wife lives in California, so do you think the L.A Visa office will be responsible for this, or should I put down Buffalo, N.Y?

Any help is appreciated.

All applications go to Buffalo first, so you select Buffalo.
