I performed my medical exams 4 days back and got the print sheet as a proof that I would need to submit.
I have one question regarding that.
On the top of the eMedical print sheet there is a space for PHOTO. However, it says "Photo not available".
So do I need to paste my photo on the sheet at the designated place or just ignore that part and upload
the document as it is.
Ideally, the clinic should've asked for your photos and one of those would've made it to this sheet. You can stick a photo there, scan the sheet and upload.

I asked one of my friends who got his medical exam done from the same place.
And now his Medical is already passed.
He said in his case also the clinic did not ask for a photo and he just pasted a photo later on his own and scanned it.
So I will do the same.

They gave the print out immediately after I registered at the front desk.
Later they did take a picture themselves before the X-Ray. So I am thinking they will use that picture
to send along with the report to CIC.
For the print out that I got I will paste a picture on my own and scan it.
Any case I will call the clinic tomorrow to confirm again.
