Medical Exam for Supervisa

Hi All:

There is a medical exam requirement for a super visa but the certified doctors in India are asking for an IMA letter to do one. What should I do? Should I apply without a medical exam? The issue is that if my mother in India does not pass the medical exam then she won't be able to come to Canada anyways. Any guidance you can provide here would be greatly appreciated.
