Please guide me the process of ECA report assessment from MCC in detail.

I have account on physiciansapply.ca.

Q.1 - what educational documents are needed to be certify to apply for source verification request? only degree or transcripts also?

Q.2 - Can i certify my educational documents and passport from the same Dean? if yes, then only 1 Certified identity confirmation form will need? or with each certified document need this form?
