So instead of posting a new topic I found this link:

It seems like for CEC Inland applicants Pre-ITA medical is feasible if you are fairly certain to receive an ITA in the coming draw. However, I called the assigned clinic on CIC website and the receptionist told me they can only do the medical after I receive the ITA. In that case, what does upfront medical mean? When can I go to the medical, directly after ITA or I have to wait until CIC send me the medical request after ITA?

I don't know the rule but I did my medcial after the ITA.
The upfront medical just means you submit it part of your PR applciation. In some streams I guess CIC would request a medical. I applied under CEC stream and upfront medcial was a requirmenet.
