lost cancelled indian passport and surrender certificate

hi, i have lost my cancelled Indian passport and surrender certificate. now i have to apply for my OCI card and i dont know how to pursue my application as cancelled passport and surrender certificate is one of the requirement. i have got my Canadian citizenship 2014. i have photocopies my cancelled passport front and last pages but no pic of cancelled page

I didn't do anything, I called few times and explained my situation but you know how unintelligent and illogical BLS employees are. God knows what are they getting paid for maybe knitting sweater or cutting vegetables on office dime. It's Indian work ethic within Canadian limits so u can figure out how it would be.
