Has anyone had difficulty linking their application to the website... it doesn't work!!!! Also has anyone been provided with the medical form but there were errors on the form and things missing? How do you get corrected and new forms. I tried calling 1-888-242-2100 but the person that I spoke to had no clue what I was saying. Please help!!!!

As regards 'medical forms', I also don't know what you are referring to. If you mean when they ask you to attend an immigration medical, you must go to a CIC approved 'panel' doctor. They will have all the 'forms' needed (almost all now complete on-line with direct submission to CIC, so no paper forms anymore).

Thank you for the response. Where my husband is from the panel physicians need the paper medical form apparently. But the form I received has two incorrect birth dates for his children who are also get the medical done and the barcode is not on it. I am at a loss.
After I linked my application, I am supposed to upload additional documents but under required documents it doesn't show any things. There is no option to upload
