Letter of Explanation required? URGENT

Hi,
My elder sister shifted to USA from UK.

1.In my previous Family Info Document, I added her UK address.
In new visa application, I added her USA address .

2. Bank issued bank balance certificate upto 29th May and suggested to upload bank balance certificate up to yesterday, in order to show balance.

Should I explain these 2 points in LOE or its not required?

I would add an explanation to any changes I would make reapplying, as, I have one rejection I don't want another by VO thinking I am doing any misinterpretation now or did in my first application and correcting now.

Have you previously applied to enter or remain in canada? should I say Yes to it , as i have UCI?

I remember putting NOs for all the questions for my Mother and then uploading it, even it was optional I did it anyways, lol.
