Hello fellow members,

Can someone please help me in determining what I need to submit as Legal Name Change Doc?
I had a different name before marriage than I have now after marriage (same surname, middle name is different).

Yes you have to produce same person affidavit in this case from notary.search this format on google will get it easily.
PR application should be from same name as its in your passport.

But I reside in USA, I believe they do not do affidavits here. What should I do?
