Job offer form

Hello
I`m working for an employer with Head Office and Main Company in Ontario but with the company in BC too where I work.
Not sure if in the pnp job form where are required to write all dates of the employer if we have to write the dates of Main Ontario Employer or BC Employer.
In my job offer is write that Main Company offered me a job but that I´m working for BC company.
My paystub are from BC employer.
SO......I'm confuse.
