None is required by the embassy itself. However, if you do think there is a discrepancy somewhere in your education profile such as a long study gap, needs an explanation for it, then cover it up on SOP etc You do not need to attach both docs at the same time. Keep it simple.

Both could be same and both are optional , they only check the documents u provide...

basically cover letter says what all documents are included in the package. and cover letter is always kept in the beginning of file.
