I worked in three companies and my roles are same except minor change.
First company - Started as Junior Software Engineer and role is changed to software engineer when i left.
Second company - Software Engineer
Third company - Associate software developer and changed to Software Developer.
Do i need to include those roles in reference letter?
I have a reference letter that mentions roles and responsibilities only for main role. it means, no roles and responsibility for junior and Associate roles. it is very minor change, and didn't change anything significantly.

If there ate no major changes in the roles in the position that you held in a company, you need not be worried, but the letter should at least mention the positions held by you and the duration of each position.
