My wife (PA, Indian citizen currently on student visa in UK) had done her medical examination and given her biometric for her UK student visa 6 months ago. She is currently in UK and was in UK at the time of our spousal sponsorship application submission (3 months ago). Do you know if she would still need to go through the medical examination in UK again and is there a possibility of exemption? OR the exemption only applies to if you had given a medical examination to enter Canada before?

She will need to do a medical again with an IRCC designated doctors.
Everyone going through the spousal sponsorship process needs to take a medical. There are no exemptions for those from the UK. The rule you are quoting is for temp visas only.
