Hi,

We applied for express entry online and now received an Imm5475 - Authority to release personal information to a designated individual. I have designated my wife but i don't understand what to fill in section of the form. ie:

Name of office where the application was submitted
Location of office

Also there are 2 fields for given names. Why ? For example if the name is XYZ where is Z is last name. What do we fill in the given names fields ? XY in one field or X in one and Y in second field.

Please help.
