Just posting this in a new thread, to make sure I get the right answer!
Is this form mandatory for all common-law relationships?
It seemed like common-sense to include it, but after reading the Guide and the document checklist 3 times, it states to complete it "only if the latter is a co-signer on the application"
It also states "if" on the actual document checklist: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5491E.pdf
Anyway can someone explain this to me? And then the next question: If I do need it, should I wait to get an AOR before sending the form, or send it ASAP? (I am thinking to wait, but just thought I would get your thoughts...)

Thanks nicelina! Looks like it is a commOn question! I wonder why they don't change the wording on the guide... Either way looks like around half people do it, and half don't without dramas for approval.
I did also reply to this in another post. I specifically asked this question to VO and they indicated that only the sponsor needs to sign the declaration if common-law partner is the principal applicant. I agree this form and directions need to be clarified. But I did send with only my signature and declaration signed/stamped at Court office. hopefully that was sufficient.
After reviewing the information on the checklist you may be okay but perhaps it might be worth sending an email to sydney just to clarify. I thought I had it clarrified but perhaps sydney misunderstood my email and the confusion arose from do we need a signature to do we need to complete form?
Thanks guys for responding! Yes that form and the fact my head is probably too big on the head photos are my only concern. I think I will wait until they are requested, just because even if I resend, I may have missed something else, and confused the process even more! I am sure manY others are in the same boat, so they will be used to receiving some applications without.
Sometimes i think it's better to just not read past threads and forget about it until you receive correspondance. i'm already addicted to this forum though
