IMM5406 confusion

Hi everyone

I have a confusion. I got ADR for my spouse to fill 5406 E. I am the primary applicant and the ADR is requested for my husband.
The email says the below
- additional info 5406 : A seperate and newly completed additional family info for (IMM5406) bearing original signatures
- additional family information IMM 5406 E

Please can anyone guide who has to fill the form. Does it have to be by me with my parents details or by my husband with his details.

Additional Family Information Form (a form completed by both the principal applicant and the spouse).

Just make sure that it is accurate, print it out and each of which sign the form hand written
Scan it then send it.
