IMM1017e medical form

i hve applied in family class sponsorshop. I got instruction for medical from embassy but they dont dont send imm1017 form
can i go for medical examination without imm1017e form.
i have also send a email to embassy but they dont reply.
plz repy me fast

I dont know where is your visa office but when I did this in Colombia, the doctor had the form with him i just had to answer his questionnaire. Also they have a list of physicians per country so just choose one and call him/her and ask about the process. They might need additional docs.

We filled out that form and took it with us to our medical exam in Manila, but the medical clinic didn't need it. They had already asked us enough questions in an email before we went so they had all the information that the IMM1017 form asks for. It's not on the CIC "Forms" main page either, so it's certainly not a must IMO....the medical clinic would likely have them on hand in any case if they think you really need to fill one out.

The doctor has to give you the form, not the other way around.

When you call to make the appointment with the panel physician, ask them about what you need to bring with you.
