I just received my IMM1017 for doing my medical exam. However, I have not received the form for my spouse who is currently residing in another country.
Could anyone please advise what to do? I suppose there IMM1017 SCL according to CIC website.

As far as I know you do not need fill any form for Medical. The panel doctor will provide all paperwork once you do your examination.
