Hello, I am preparing the application for sponsoring my wife from India. We do have a problem with her passport. Both her first and last name are under GIVEN NAME and her SUR NAME column in the passport is blank. So, how do i fill the application form now? Leave the FAMILY NAME blank or should I write her last name there? Not sure what to do here. If i follow the way it states (name as in passport), the FAMILY NAME column will be blank. Please advise. Apparently it is quite common in India to have names in passports like this.
Hey i had the exact same issue! I just included the name exactly the way it is in the passport. For eg. if the family name is not given and in the given name it is 'Anita Raju', then on the forms, you leave the family name box blank, and instead enter the given name/names as 'Anita Raju'.
