IMM 5710 question 3 which one do i choose?

I am applying for one or more of the following:

1. a work permit with the same employer
2. an initial work permit or a work permit with a new employer
3. restoration of your status as a worker
4. temporary resident permit

right now my situation is i am sponsoring my spouse in family class.
She has work permit and it end DEC 2016 which is in 6 months.
She is working now, temporarily she is not going to change her job, but next year she might change(if she get the spouse open work permit)
I want to submit this form with all of my other documents for SCLPC class.
which one should I choose?

I just want to ask in my situation which of these four should i choose?

The OWP with inland SCLPC class is always "initial work permit" when you first get it, as it is a different category from all other work permits.
