Imm 5645e form mistakes

I recently applied for a visit visa Canada via online, unfortunately I just found that I made a big mistake on imm 5645e form , I did not include my dads name and moms names to matters worse even my name is not included and I didn’t put date , would this affect my application, am really stressed honestly.
