IMM 5540 and applicant photo req

Hi All,
I have a questions regarding the form IMM 5540 "Sponsor Questionnaire".

On question 9 it asks which canadian visa office issued your immigrant visa/confirmation of PR? what to put in here. The one time entry visa I got says issued at Buffalo and if I remember correctly I send my PR application to Sydney, nova scotia on Dec 15, 2011. so what to put in here?

and the 9 photo(s) of her, Do I need to ask her to write anything on the back like name/signature? I didn't find the instruction, may be I missed it.

Are you applying outland?
Where is your wife from? This will tell you which VO processes the country she is in.

For the photo, the instruction should be in your country specific checklist.

yes, applying outland. She is in Bangladesh.
But this question I believe IMM 5540 is for me as I got my PR and live in Montreal. For her I know its Singapore office.
I remember, I sent my PR application to sydney, Nova scotia and when I got my passport in that one time entry visa it says Issued at Buffalo. Now they are asking me which Canadian office issued my immigrant visa. I did my landing by land Lacolle border of montreal.


In the country specific check list it says :

Supply nine (9) recent photos taken in the last six months for you and each family member. Follow
the instructions in your guide (see Photos in section on completing the Application for Permanent
Residence in Canada) and in Appendix B: Photo Specifications

I know the specification but does she need to put her name/signature on the back of all those or not I couldn't find in the guide 3999
