IMM 5533 Parts????

Hello all. We sent in our papers, and paperworks changed so we need to resend in.

Im looking at IMM 5533

It says to please include unt checklist, Both parts A and B, well the form shows B in the uper right corner, and in my searching trying to find part A but no luck. Is it something im missing or is it a different form somewhere.
Any help be appreciated,
