Question 4 asks if you are not living together, have you visited each other during your relationship?
My Question:
1. Since Question 4 does not apply to me, do I need to leave the YES/NO boxes blank and leave a "Not Applicable" message in the space provided?
2. Do I also leave the details section of Question 4 blank or leave a "NA" message?
