Hi,

I have a few questions regarding the IMM5409 (common law). I just don’t want to make any silly mistakes.

1) Can I print most of the text and fill the rest (like names) in black ink? Are there any rules?
2) I have nothing to put into the province/territory/state field - can I leave it blank?
3) The last date of the relationship is the date when the form is certified?
