Yes u must ... if you cant type it, print the form with all information and write all names with black-ink pen (just below the names in english).

What i am able to figure out till now is one form is filled by me and one my wife : ee

Now question :

1)If I am filling 5406 will the below example is good to go ? Assume my name is amar and my wife is laxmi

signed by me
Now When my wife fills her copy how it will work ? Assuming her parents are X and Y

This is how I understood ( per the guide)...this form needs to be filled individually by principal applicant, spouse and dep children 18 + he

when I fill this form...my name should be applicant and I need to provide all my relevant details...
when my wife fills this form she will be the applicant ( co-applicant) and my name will come as spouse and her parents and siblings details ( children info will be same for both of us)

Even I thought so, that both primary applicant and spouse will be applicant while filling out their own forms.....May be seniors can clarify on this doubt......Seniors please reply
