IMM 1344 BARCODE PAGE

Hi - When I print the IMM 1344 barcode page, there is not a space for the signature as the instructions outline. Do I just sign page 6 of 7 where there are signature fields or do i also need to sign the barcode page?

Sign only where it says Signature. Make sure both Sponsor and Sponsored person sign in the places as indicated
