IMM 0008 DEP Additional Dependents form

Hello,
I am in dire need of some help a clarification with filling out this form, the situation is that I am in a Common-Law relationship with my Canadian Girlfriend, we have two children together and I have only this form to finish.

It says on the Document checklist that I need to fill this out but when I reach Q12 I have no idea what Dependent type that I am, I have tried to call the helpline but all that I get is put on hold and then then phone hangs up after half an hour, have been to a Service Canada but they have said to call.

I am 35, A fully trained HGV/AZ licence driver so I can provide for myself.

Any help would be most appreciated

That form is if you have more than 5 dependents (children, or children of your dependent children). Do you have more than 5 children or grandchildren? If not, then you don't need to fill this form out.
