I'm an international student studying in Canada right now.. I want to renew my visa for a trip abroad in summer. Does any body know where is the application form?
I have my study permit and CAQ expired in 2013.

You should really do a search, since the question is answered at least twice every day. You apply to a Canadian office in the US by mail. The Buffalo instructions are here: http://tinyurl.com/4h3fsnz

What is the expiry date on your visa and study permit?

my visa expired on 31 aug 2010, study permit on 31 aug 2013.

Do I need to provide financial proof when I apply for visa?
