Because I have to add some information I didn't properly wrote, I am looking at an issue that might (or not) be raised by the IRCC agents.
This goes for PAs coming from countries where it is usual that a married woman takes the name of the husband, or adds a "-" and combines the two last names.
When enumerating all the family members (IMM 5406) (web and/or paper), it is asked about the PAs parents names.
If you are John Doe, father name Jack Doe, how would you report you mother's name?
1. Mary Doe (as it is written on your birth certificate)?
2. Mary Doe (born Smith)?
3. Mary Smith (married Doe)?
Personally, I would go with option #2, but you never know.
And I would mention it as such everywhere I need to enter my mother's name.
