I've received my COPR and I'm planning to arrive to Toronto in coming new few weeks. In my COPR it is mentioned as Condition No: 32 - "Must report for medical examination,surveillance or treatment and at specified times and places". This as because I had an inactive TB in my medical history.
I'm coming for only 2 weeks to Canada for soft landing. So it will be very important for me to get the medical surveillance done before I leave. The arrival destination for me is Toronto. Is there any reference links where I can look for an appointment and book the same well in advance so that I can plan the soft landing accordingly?
Just call them when you land. You will have to call them again before you leave to say that you're leaving Canada.
They will instruct you what to do when you call. They may very well likely subject you to a TB test, which you can't do in two weeks. You also can't do it in your home country. They will make you do it when you've settled permanently.

You may be flagged every time you enter Canada and report to CBSA secondary. You just have to explain about your medical surveillance. It shouldn't be a big issue, just explain your situation.
You should also complete the surveillance within your first 5 years of landing.
Just explain everything truthfully to the health officials (that you are doing short landing) and the CBSA (that you are in touch with health officials). This is all perfectly allowed.
