Help with ADR (visa refusal and disability)

Friends, I need your input. I got an adr(further explanation) for previous visa refusals. I have written the ones I remember but there are some I don't completely remember.For example the reason and exact date. I only remember the year.How do I address this?

Also I have a medical condition(physical disability) for which I don't receive anymore treatment because its not necessary and will not require any more treatment in the future.It is mild and doesn't affect my life. I received an adr(further explanation) but because I disclosed it for the statutory questions but I passed the medical examination so I am not sure how to address it.I disclosed it just to be on the safe side.
