1. Past Travel History - I have made 50+ trips in the past 10 years and some of the immigration stamps are really unclear. How does one cover this? Would partial travel history suffice? Is there a way to simplify this?
2. Police Verification - I have lived in 6 cities in the past 10 years (within and outside of India). Would police verification from the current city of residence suffice? (in India). Also, has anyone got a police verification certificate done from Dubai? (I used to reside there more than 5 years ago)

Try and be as meticulous and comprehensive as possible and attach an LOE where you're unable to do that.

PCC is for the country so procuring it from the authorities at your current city/state/province of residence would suffice.
Below is the link for an agent who can help you obtain UAE PCC from India or any other gulf countries. I have obtained my UAE PCC through them only and I received it within 20 days here in Bahrain.
Also, just one last question - do I need to provide police clearance certificates for each state in India I've resided in? I am unsure of how it works as the only state which allows for an application online is Delhi.

No - PCC is for the entire country. There was a fairly active thread recently where members discussed getting this done via passport authorities anywhere in the country - do a quick forum search.
