Help plz minor application

My application was returned because I attached representative form for my minors as well. And I accidentally put my name on “applicants name” but now I realize I don’t need to send representative form for minor as they don’t need to sign that form as they are under 18. But do I need to print new application form for them as in the minors application on page no. 6 section 11 “Appointment Of A Representative “ do I need to stay with “Yes” for part c? Or shall I type the form again and type “No” this time for section 11 page 6 in minors application? M confused
