Hi,
Yesterday, i received a request from CIC to submit IMM5669E (schedule-A form) in 7 days. Appreciate, if someone can share sample filled form as having couple of queries.

Also appreciate reply on the following queries -
1. In education section, unable to type complete university name and degree name
2. In personal history, unable to type City and Country name. hope abbreviation is fine here like - for Canada, CA and for India, IN ?
3. What should i write in status in personal history when i am in foreign country on Work permit or Tourist visa
