Can somebody tell me what should be the "Date and place of your last entry to Canada"

Is it when I landed to Canada when I came from back home or even the 2 day USA visit is considered to be the last entry in canada

Yes .. any overnight stay outside Canada needs to be specified.

It should be only specified in the supplementary travel form or it is considered as last entry

Its last time you entered Canada. Yes so if you visited States for 2 days and then came back to Canada then consider that your last entry.

Both. You have to mention it on Supplementary Travel form that you visited States and after visiting States when you entered again in Canada that date will be last entry date.

Don't leave any gap, so disclose everything even if it's a 1 day tour.
