Hi There,
My application returned today because It was missing my birth certificate. It is my stupid mistake and I forgot to attach it to my documents.
I have now attached my birth certificate and wonder if I need to fill in all the forms again? I see a stamp on IMM0008 with the date which was received by CIC Sydney office.

I faced a similar situation.........I sent the same forms back and attached a cover letter explaining the situation

No, there is no need to redo the application forms. I was also in a similiar situation. I just attached the information they were missing and I also sent a copy of the letter they sent me when the application was returned. I have since received my AOR. Best of Luck

Thanks Jes, also if you can advise that is it a must to attach an affidavit if I need a translation of my birth cert as I have certified copy of it and also the tranlsation all attested and notarized. Do I still need affidavit ??

Not sure what you mean by "attested" but yes, you need an affidavit from the translator (See the instruction guide for a definition of an affidavit). The only exception to that requirement is if the translator is one of the "certified members in good standing of one of the provincial or territorial organizations of translators and interpreters of Canada." - in that case, s/he would not need to supply an affidavit.
Thank you so much guys. I have now attached a copy of my birth certificate and going to send it with my original documents again this friday. Hopefully I get my AOR soon.
