Help! Application Returned

Hi, I just got my application returned from CIC. On the letter, it says photo and IMM0008 are missing/incomplete. I forgot to write down my name and DOB on the back of photo ( I was careless). Also, I used an outdated IMM008 form (they updated in Dec 2016, I used the Aug 2016 version). I think these are two mistakes I made. But before I resubmit it, I still want to get some thoughts from you guys and make sure that is the root cause.

Also for those who haven't submit your applications, double check the checklist and make sure you capture these details.

Sorry to hear you got your application returned. They will definitely send it back if you used outdated forms. Maybe get a friend to look over the checklist and make sure all your paperwork is there. I sorted mine by the checklist so it was easier to see if something was missing. Hope that helps and you are able to get it back to them in a timely manner. Double check too that you got your whole application back and nothing is missing. Don't beat yourself up too bad, we all make mistakes and there are a lot of applications being returned lately.
