It doesn't work that way. Regardless of whether the wife is sponsored or immigrates independently, she will have to include the children in her application and they will have to be medically examined as part of her immigration process. They don't have to come with her immediately - but she must include them in the application (along with their birth certificates / passports) and they must pass the medical. In other words, she will have to declare her children immediately when she applies (not later).

Yes - but if your brother sponsors her - CIC will see the children on her application and this will put his own PR status in jeopardy since it will show that he lied when he landed and failed to mention that he has children. This is misrepresentation. Misrepresentation can result in having your PR status revoked.

So if your brother goes forward with his plans and tries to bring his wife to Canada, he should be prepared for the fact that he could lose his own PR status in the process.

What you don't understand what scylla is trying to tell you is that when the wife goes to apply, she will have to list her husband and children on the application. That's the point that could "throw up the red flag". At least, that's my understanding.
