My Name is "Geo G Mathew" in many documents since in India it's acceptable to use initials instead of full names.
But in My passport and other documents, Its Like "George Gavi Trek Mathew" in which the middle name is my family name which is appeared as initial in another document.
SO Should I mention this as another name? If so, Should I give any explanation for the same while applying for CIC?

The name stated in your passport and birth certificate matter. That said, you must state all other names, aliases, nicknames, change in names (whatever is applicable) in the visa application
