We put the following:
Family Name: Doe
Given names: Jane
May
It's for the Sponsorship Undertaking where he lists who he's undertaking... I had put Jane May on my documents - but now hubby is asking and I'm doubting myself.

I was always taught that given names were first and middle!
These forms are SO much WORK! They really test your patients and your reading comprehension! LOL

I did some googling before I posted here and it all said "Jane".
For family name, lol, did you use your maiden name or your married name?
Heather is correct family name is your last name given names are your first, middle, secon, third.
EX,,,, PETER KYLE CUNNINGHAM
CUNNINGHAM is the family name
PETER KYLE is the given manes
EX,,,, PETER KYLE KENT CUNNINGHAM
CUNNINGHAM is the family name
PETER KYLE KENT are given names
EX,,,,PETER KYLE KENT THE 3RD CUNNINGHAM

I used married everywhere it said family and my maiden only when it asked for maiden or name at birth. I think that I must have filled out the forms about a dozen times. I printed and hand filled and then transferred the info over after. Husband and I actually almost missed signing some of the documents as the whole process became an obsessive quest to make sure they were filled out right.
So in the case of my mother-in-law when he's listing her, it asks family name. He still would put 'Doe' because she's not remarried to anyone after his dad passed away.
When forms ask for your 'given name(s)', put your first and middle names. Whatever is on your passport is safest.
On another application it asked for it again - and it indicated "shown on passport"
Wish these forms were more universal in their consistency!

what about the additional family?

but at the end the additional family asks questions about ur family

thats fine. u have to write ur father mother brother sister names. its must
